Become a member of Isaac Health’s GUIDE Program

You must be enrolled in Medicare Parts A and B to be eligible for the GUIDE program. If you are not enrolled in Medicare, reach out to our Care Team to learn more about how our other services can support you. We work with most major insurance plans for non-GUIDE services.

Receive comprehensive dementia care and support as part of your Medicare benefits

The Guiding an Improved Dementia Experience (GUIDE) model is new program from Medicare (CMS) that supports Medicare beneficiaries living with dementia, as well as their families. Isaac Health is a proud participant in the GUIDE Model right from the launch in July 2024. 

As a member of GUIDE, you and your loved ones will have access to a dedicated medical team, caregiver support, and respite care, among other supports and services covered by Medicare. At the center of the model is your personal Care Navigator, who will guide you throughout the dementia care journey.

Around-the-clock support

Isaac Health is always there for you and your family. As a member of our GUIDE program, you will have access to dementia-trained specialists and support staff through our 24/7 support line.

A personal care navigator

Your care navigator will be your personal guide through the whole journey and will check in with you or your loved one regularly. They can also make referrals to community support organizations.

A dedicated medical team

Our specialist doctors and nurse practitioners will perform a detailed assessment upon enrollment in GUIDE and at least once a year. Your medical team will create a personalized treatment plan for you or your loved one.

Caregiver support

We host caregiver training sessions for our GUIDE members. Our specialized caregiver coaches also provide 1:1 sessions to go over what dementia is and what it may mean for your family. Additionally, we run support groups and group activities for members and families.

Respite care

Everyone deserves to take a break. Respite care allows caregivers to take a temporary rest from caregiving, while a trained home care aide comes to your home to look after you. Through our GUIDE Program, you will have 24 annual slots for respite care.

Fully covered by Medicare

If you are enrolled in Medicare Parts A and B, you can receive all the above benefits as a member of Isaac Health’s GUIDE Program – at no cost to you.

To be eligible, you must be enrolled in Medicare Parts A and B with Medicare as your primary insurance. You are not eligible if your primary insurance is Medicare Advantage or another Medicare health plan (including Special Needs Plans and the Program of All­ Inclusive Care for the Elderly). You are not eligible if enrolled in Medicare hospice benefit or if you are a long­-term nursing home resident (defined as a nursing home stay of 4 months or longer and not covered under the Medicare skilled nursing facility benefit).

If you are not enrolled in Medicare but are still interested in receiving care for yourself or a loved one, reach out to our Care Team to learn more about how Isaac Health’s other services can support you and your loved ones. We work with most major insurance plans for non-GUIDE services, which include assessment, treatment, and care management for dementia.

If you are eligible for GUIDE, you will be fully covered by Medicare for all services you receive through the program.
