CMS Launches New Dementia Care Management Program

CMS Innovation

The Centers for Medicare & Medicaid Services (CMS) has just launched a new dementia care management program called GUIDE (Geographic Variation for Individuals with Dementia and their Caregivers). This program is designed to improve the quality of care and support for people living with dementia and their caregivers.

GUIDE is a pay-for-performance program that will provide participating organizations with a per-beneficiary, per-month payment for providing care management and coordination and caregiver education and support services. The program will also pay for a defined amount of respite services for a subset of model beneficiaries.

Isaac Health is a leading provider of dementia care management services. We are committed to providing high-quality care and support to people living with dementia and their caregivers. We will be excited to apply to participate in the GUIDE program and we believe that our experience and expertise, as well as the evidence we have already collected, will make a significant contribution to the program’s success.

We encourage Medicare ACOs and primary care groups to contact us to discuss making use of the new GUIDE model. We are confident that we can help you improve the quality of care and support for your patients living with dementia and their caregivers.

Here are some of the key features of the GUIDE program:

  • It is a pay-for-performance program, which means that participants will be rewarded for providing high-quality care.
  • It is a comprehensive program that includes care management, coordination, and caregiver education and support services.
  • It is a flexible program that allows participants to tailor their services to the needs of their patients and caregivers.

If you are interested in learning more about the GUIDE program or collaborating to participate in the program, please contact us today.

We look forward to working with you to improve the quality of care and support for people living with dementia and their caregivers.
