Isaac Health is an accepted participant in CMS GUIDE Model Program

NEW YORK, New York, April 9, 2024 – Isaac Health, a leading national provider of brain health and dementia care, has been accepted in the established and new model tracks of the ​​Centers for Medicare & Medicaid Services’ (CMS) Guiding an Improved Dementia Experience (GUIDE) Model. 

The GUIDE Model focuses on enhancing care for individuals living with dementia and their families through advanced care management, respite care, and caregiver support. As a GUIDE Program, Isaac Health’s team of clinicians and care navigators will provide patients and their caregivers with personalized treatment planning through telehealth, advanced care coordination, caregiver support, respite services, and a 24/7 hotline in alignment with the GUIDE Model.
